COVID Case Report
This submission must be completed weekly by district personnel.
- Your district Superintendent received an email with the link and authentication code on Friday, August 28th.
- The first submission should go back to your first day of school and include all on-campus positive cases.
- Districts that do not have any positive cases do not need to complete the submission for that week.
- For weeks in which you do have positive cases to report, you must complete the submission by 5pm on Monday for the prior week.
- Online web form to be submitted weekly beginning September 8, 2020

Below are the questions that need to be answered to complete this submission.
Section 1: School Identification
- Is this report for cases on a single campus or for cases that impact multiple campuses? Choose: Single Campus or Multiple Campuses from the dropdown list. The Multiple Campuses selection is only intended for the reporting of staff who work at more than one campus, such as speech, dyslexia, or intervention specialists. The Multiple Campuses selection is only intended for this purpose.
- If you choose Single Campus, you will be required to enter your district’s TEA Campus ID #- Begin typing your District/campus number and choose your campus from the dropdown list.
- If you choose Multiple Campuses, you will be required to enter you District’s TEA District ID #- Begin typing your District number and choose your district from the dropdown list.
- LEA Authentication Code- This can be found in the email sent to the district Superintendent.
- Reporter Email- The email address of the staff member completing the submission
- Reporter Phone Number- The phone number of the staff member completing the submission
- Reporter Comments- If applicable
Section 2: Case Information
- What is the total number of new COVID-19 cases the school was notified of during the previous week? Report all new COVID-19 cases that the school was notified of the previous week. You may report a maximum of 20 cases per form. Please submit additional COVID-19 school report forms for any additional cases. Please ensure the total number of cases reported on all forms for the previous week equal the total number of cases the school was notified of during the previous week.
- How many of the total number of new COVID-19 cases are being reported on this form? Choose a number between 1 & 20 on the dropdown list. This number will indicate how many individual cases will populate below.
Section 3: Case 1- You will fill out the following information for every case you indicated (1-20) in the question above.
- Case 1: Date on Which Confirmed Case Notification Was Received- MM-DD-YYY
- Case 1: Notified By- Choose either Individual or Public Health from the dropdown list.
- Case 1: Case Individual Type- Choose student, staff or other (on-campus close contact occurred) from the dropdown list.
- Case 1: Was the positive case contracted on campus or off campus? – Choose On campus, Off campus or Unknown from the dropdown list.
- Case 1: School specific contacts identified? Choose Yes, No or Unknown from the dropdown list.
Click Save and Close if you are finished or click Save and New if you have more then 20 cases that week.
